8Steps:
91) With the serial number of the console, go to sony's technical support page and fill out the form for a warranty check.

112) Once you filled out the form, you'll get a confirmation email
123) After a few days, you'll receive a box where you will put your "ps4" in.

14Here's what I did:
15a) Filled out the pink slip but did not attach a receipt (even though they say they need it)
16b ) Ripped one of the foam thingies in half
17c) Crumbled up both the plastic wrap and the pink slip
18d) Taped the box using the tape they provided
19e) Cutting the tape and then re-taping it with another tape

214) Go to the FedEx website and fill out a form for "Pick-up"
22(I prefer pick-ups because the carrier person won't question the big, light box. But the lady at the front desk might.)
235) After FedEx picks up your package, you'll get another email in a few days confirming that they have received the package.
246) They will send another email asking to contact them.
25(I went to their livechat because they apparently "called" me)
267) After talking to the representative, they will send another email stating that they will launch an investigation.
278) After about 15 days, they'll send another email stating that they have approved for the replacement.
289) Then they will send another email with tracking details.
